Prior to your scheduled Telehealth "The General Patient Consent Form" must be completed in ink, signed and returned back. A Copy of your Drivers License is required for identification purposes as well. (Can be sent back via email or fax) For NYSHIP, there is a $25 Copay that is collected. Telehealth visit takes approximately 5-10 minutes over the phone to go over your medical intake. Upon completion, I write your Medical Massage Prescription and fax/email it directly to the office you are receiving services. The Medical Massage Prescription or PT precription is valid for 8 weeks, in which we then complete a brief follow up Telehealth visit.

In person Medical visit to address any prescription renewals, nutritional counseling, necessary referrals, ordering blood work, blood draw in office etc. Specialist Copay is collected for your Medical Visit.

Introduction to Botox and Filler 8 hour course with certificate of completion and 8.25 State CME’s hours. Class consists of an in-person 8 hour training, review of anatomy, Botox and Filler, purchasing products and live 1:1 injecting Botox and Filler. Deposit is nonrefundable if cancelled 1 week prior to the class date.
