Introduction to Botox and Filler 8 hour course with certificate of completion and 8.25 State CME’s hours. Class consists of an in-person 8 hour training, review of anatomy, Botox and Filler, purchasing products and live 1:1 injecting Botox and Filler. Deposit is nonrefundable if cancelled 1 week prior to the class date.

1:1 Botox Training (& Filler if needed). Botox/Filler supplies can be provided. *Prices for product are separate from the training cost. Education on Toxin/Filler, review of anatomy, connecting with reps, purchasing products/supplies. Models can be provided to inject Toxin (& filler).

Botox Consultation/Treatment

Follow up to reassess your botox approximately 2 weeks after your initial visit.

Botox is injected into both armpits to treat Hyperhidrosis “excessive sweating”.

Reassessment for your filler within 2-3 weeks of your initial appointment.

VI Peel treatments are an effective solution to common skin issues, including pigmentation, acne, scars, and fine lines. This chemical peel can also prolong your botox! Application takes approximately 10 minutes and you are provided with a post treatment facial kit with post care instructions.

Prior to your scheduled Telehealth "The General Patient Consent Form" must be completed in ink, signed and returned back. A Copy of your Drivers License is required for identification purposes as well. (Can be sent back via email or fax) For NYSHIP, there is a $25 Copay that is collected. Telehealth visit takes approximately 5-10 minutes over the phone to go over your medical intake. Upon completion, I write your Medical Massage Prescription and fax/email it directly to the office you are receiving services. The Medical Massage Prescription or PT precription is valid for 8 weeks, in which we then complete a brief follow up Telehealth visit.

In person Medical visit to address any prescription renewals, nutritional counseling, necessary referrals, ordering blood work, blood draw in office etc. Specialist Copay is collected for your Medical Visit.

Weight Management Follow Up

Existing patients- Weight Management Telehealth visit via Facetime or Audio call for those who live far or cant make it to their routine in-person follow up. Medication Shipment is $15.00 in addition to your monthly medication. Option for medication pick up is available.
